Submit a Claim Form - Claim Instructions

To qualify for a payment from the Net Settlement Fund, you must submit a timely and valid Claim Form. The Claim Form must be postmarked or submitted no later than August 5, 2026.



How to File by Mail:

Fill out, sign, and date the Claim Form. Then mail the Claim Form to the following address:

Please keep copies of everything you submit, as your claim may be audited, and you could be asked to provide additional information to process your claim.
